IMPORTANT NOTE: While the trustee/custody details have not yet been finalized, your records will remain available to you after our office has permanently closed. If you do not need immediate access to your records, we suggest that you delay your records request. The most efficient method for records requests is for your new office to request those records while they are preparing for your next visit with them. This allows the records to be handled by fewer parties and eases the load on our limited staff.

Records Release Information
**Your records are now in the custody of Farr Eyecare Group.** Their office is three miles south of ours, also on Alma School Rd. We encourage you to continue care with their office. No records release is required to establish care with their office. Kindly inform them that you are a previous patient of Horwitz Vision Center when you schedule your appointment.
We expect a high volume of records requests. Therefore, we recommend that you request records be sent directly to your new provider via fax or picked up by yourself personally.
